Preventing risks and side effects of testosterone replacement therapy (TRT)

Testosterone Replacement Therapy (TRT) can be an effective treatment for men with documented low testosterone levels and associated symptoms. However, like any medical treatment, it carries potential risks and side effects. To minimize these risks and ensure the safety and effectiveness of TRT, men can take several important steps:

 1. Get a proper diagnosis and consultation

  • Confirm testosterone deficiency: Before starting TRT, it's essential to have your testosterone levels accurately assessed by a healthcare professional, ideally through multiple morning blood tests.
  • Discuss underlying causes: Your doctor will investigate other potential reasons for your symptoms before recommending TRT, according to Harvard Health.
  • Engage in shared decision-making: Discuss the potential benefits, risks, and uncertainties of long-term TRT use with your doctor to make an informed decision. 

2. Careful consideration and monitoring of existing health conditions

  • Pre-existing conditions: Certain health conditions, such as untreated heart failure, untreated obstructive sleep apnea, a recent history of heart attack or stroke, and elevated red blood cell counts, may make TRT unsafe and require cautious consideration or avoiding the therapy altogether.
  • Prostate health: TRT may stimulate the growth of existing prostate cancer, so men with a history of prostate cancer or those at high risk should be closely monitored and may need to avoid TRT.
  • Regular monitoring: Ongoing monitoring is crucial while on TRT. This includes:
    • Testosterone levels: Regular blood tests to ensure levels remain within the normal range.
    • Hematocrit: To check for increased red blood cell production, which can elevate blood clot risk.
    • PSA (Prostate-Specific Antigen): To monitor prostate health and potentially detect prostate cancer early.
    • Bone density (DEXA scan): Regular checks, especially at the start of treatment, to rule out osteoporosis or osteopenia.
    • Liver enzymes: May be monitored, particularly with oral testosterone formulations or in individuals with pre-existing liver conditions.
    • Physical examinations: Regular check-ups (e.g., every 3-6 months) to evaluate blood pressure and other potential side effects.
    • Cardiovascular risk factors: Assessment before and during TRT. 

3. Adhere to treatment guidelines and lifestyle modifications

  • Strictly follow prescribed dosage and administration: Taking testosterone only as prescribed and avoiding sharing the medication are critical for safety.
  • Lifestyle choices: Healthy habits, including adequate sleep, regular exercise (especially weight training), a balanced diet, stress management, and avoiding excessive alcohol and smoking, can support TRT effectiveness and reduce potential side effects, according to Kaiser Permanente.
  • Delivery method considerations: Discuss the pros and cons of different testosterone delivery methods (injections, gels, patches, etc.) with your doctor, as side effects can vary depending on the chosen method.
  • Address estrogen levels: Testosterone can convert to estrogen, and higher body fat can contribute to this conversion. Monitoring estradiol (estrogen) levels and potentially adjusting the dosage or using aromatase inhibitors may help manage side effects like breast enlargement.
  • Manage side effects proactively: Openly communicate any side effects or changes you experience with your doctor, as dosage adjustments, alternative medications, or other strategies can often help manage these issues. 

By following these recommendations and maintaining close communication with your healthcare provider, you can significantly reduce the risks associated with testosterone therapy and optimize the potential benefits.
